EXHIBIT 2 <br />B. Describe if your organization will provide case management, job retention services, <br />mentoring and ensure the participants attains their goals. <br />Case management will continue to be provided to ensure challenges are identified and services <br />implemented forjob retention. Mentoring and tutoringwill also be provided as needed to ensure <br />goals are achieved. <br />C. Describe how your organization will ensure that youth meet the performance indicators <br />during the follow-up period. <br />Case managers keep contact logs on exited participants and file monthly reports and quarter <br />reports on status, barriers identified and individual progress, Case Managers also during the <br />follow-up period are sources of encouragement for the participant to seek higher education or <br />develop new attainable goals for continued success. Documentation to provide evidence of <br />performance includes but is not limited to: pay stubs, direct contact with employers, participant <br />probation or employment reviews. Communication is a key component to the follow-up services <br />and consistent reminders that OCCC support services will continue to be available to assist with <br />needs. <br />D. Describe if your organization will provide youth with incentives or supportive services <br />during follow-up to assist youth retention. <br />Santa Ana WIOA participants will receive follow-up services for twelve months after they finish <br />the program. Their case manager will contact them once a month or more, as needed. They can <br />receive monthly transportation services: a bus pass or gas card. They will receive job leads from <br />both their case manager and the Corps -to -Career Specialist, as well as resume help and other <br />work readiness assistance. They are eligible for help with interview outfits and/or work uniforms. <br />In addition OCCC will also refer those who qualify to the Americorps scholarship program. The <br />OCCC will also refer them to other organizations for additional services. <br />E. How often will the youth be engaged or receive services during the follow-up period. <br />At a minimum our case managers make contact monthly but are available on an as needed basis. <br />15. WIOA Performance Indicators <br />A. Include a flow chart or visual aid or brief explanation of how your organization will meet <br />the following WIOA Performance Indicators: <br />See the attached flow chart immediately following the narrative to the questions below. <br />1.
