COMMUNITY DEVELOPMENT AGENCY <br />HOMELESS SERVICES <br />PROGRAM <br />01118811 <br />To utilize a multi -disciplinary approach to address homelesssness, especially concerning; health and safety conditions, <br />provision of outreach and engagement activities, and collaborative work with community partners to address homelessness <br />in Santa Ana. The guiding principles include: <br />Goal of bringing bestpractices ofhomeless service delivery to the City of Santa Ana, such as the Housing First and <br />Harm Reduction model. <br />* Strengthend collaboration with the County of Orange and other cities in coordination of homeless services and <br />* Increased assistance to individuals experiencing homelessness and addressing areas ofhousing, employment, <br />substance abuse treatment, medical health and other basic needs. <br />* Coordinate services with law enforcement agencies, affordable housing, homeless outreach, housing navigation, and/or <br />case management. <br />SERVICE PROGRAM <br />Activities implemented throughout the City to address homelessness will be coordinated by the Commumiry Development <br />Agency. In Fiscal You 2018-2019, activiftes will include: <br />1. Supporting the permanent and long-term solution of Permanent Supportive Housing; <br />2. Strengthening the collaboration with the County of Orange, other cities, and stakeholders in the coordination of homeless <br />services and the development of a regional plan, <br />3. Developing and implementing a city-wide strategic plan to address and reduce homelessness. <br />a. Participate and provide leadership within the Orange County Coordinated Entry System, Continuum of Care Board, and <br />Committees. <br />2-141 <br />
