y <br />City of Santa Ana <br />ITEM 12 AGREEMENT TO CONDITIONS <br />Agreement to Conditions <br />Additional Information <br />The Proposer agrees to promptly supply the CITY OF SANTA ANA and OCFA with any <br />further information requested for the accurate determination of the prospective Proposer's <br />qualifications to perform the proposed services. <br />Right to Audit <br />The Proposer agrees to give the City, County, and/or the CITY OF SANTA ANA and <br />OCFA the right to audit those documents or records' (including financial records) <br />requested in the Submission Form, provided by the Proposer, or referred to in the <br />Proposer's submission prior to and after award of the contract. <br />Agreement to Pay Expenses Required <br />THE CITY OF SANTA ANA or OCFA may require Proposers submitting proposals to <br />participate in an onsite visit by the CITY OF SANTA ANA and OCFA staff to inspect and/or <br />verify records. If based outside of Orange County, the Proposer submitting the proposal <br />agrees to pay related travel expenses,. including food and lodging, for up to three the CITY <br />OF SANTA ANA staff members and three OCFA staff members for up to two days to <br />conduct the inspection. <br />Agreement to Expand Resources as Needed <br />The Proposer agrees to expand its resources, at the sole discretion of the CITY OF <br />SANTA ANA or OCFA, and at no additional cost to the City, including number of <br />ambulances, if response times are not met or if call volume increases. In addition, the <br />Proposer agrees to maintain, at a minimum, the number and type of vehicles as proposed <br />throughout the term of the contract. <br />The CITY OF SANTA ANA is in the process of annexing parcels of land owned by the <br />COUNTY OF ORANGE that would be incorporated by the CITY OF SANTA ANA. The <br />area is commonly referred to as the "Seventeenth Street and Tustin Avenue Island". The <br />area consists of the following: <br />29 single family homes: 13942-13971 Deodar Street <br />20 single family homes: 13822-13831 Medford Street; <br />17152, 17162, 17182 Medford Street <br />8 business locations: 2301, 2305, 2307, 2309, 2311, 2313, 2315, 2401 <br />Seventeenth Street <br />3 business locations: 2301 Seventeenth Street <br />7 business locations: 13912 Ponderosa Street <br />City of Santa Ana Page 181 <br />Fire/EMS Emergency Ambulance Transportation and Related Services <br />Request for Proposals: # 18-059 <br />August 28, 2018 <br />W M71 <br />
