POLICY HOLDER NOTICE <br />CERTIFICATE HOLDER CANCELLATION NOTICE SCHEDULE <br />Should this policy be cancelled before the expiration date hereof, the producer will <br />endeavor to mail 30 days written notice to the certificate holder named herein, but <br />failure to do so shall impose no obligation or liability of any kind upon the Insurer, <br />the producer, or the respective agents or representatives of each. <br />SCHEDULE: <br />CERTIFICATE HOLDERS AS IDENTIFIED ON THE MOST RECENT QUARTERLY <br />SCHI DULE OF CERTIFICATE HOLDERS PROVIDED BY THE INSURED'S BROKER OF <br />RECORD TO THE INSURER. <br />
