Seismic Certificate of Applicable Code <br />I am a State of California licensed structural engineer, civil engineer or architect and certify that the building <br />located at; 801 Civic Center Drive, Santa Ana, CA 92701, was reviewed by me. This certification was either <br />prepared by me or the bulk of work was performed under my direct supervision, I have no ownership Interest <br />in the subject property. <br />A Certificate of Applicable Code may be provided if the entire building was constructed under a permit <br />approved by the local jurisdiction and was designed to meet one of the following requirements: <br />Ell 998 or subsequent editions of the California Building Code; or, <br />X 1976 or subsequent editions of the Uniform Building Code and the building does not have any <br />one of the enumerated characteristics or conditions listed below: <br />• Unreinforced masonry elements, whether load -bearing or not; not including brick veneer; <br />• Precast, prestressed, or post -tensioned structural or architectural elements, except piles; <br />• Flexible diaphragm (e,g., plywood) -shear wall (masonry or concrete) structural system <br />constructed pursuant to editions of the Uniform Building Code prior to the 1997 edition; <br />• Apparent additions, alterations, or repairs to the structural system made without a building <br />permit; <br />• Constructed on a site with a slope with one or more stories ap rtiall below grade (taken as <br />50% or less) for a portion of their exterior; <br />• Soft or weak story, including wood frame structures with cripple walls, or is construction over <br />first -story parking; <br />• Seismic retrofit of the building, whether voluntary or mandated, whether partial or complete; <br />• Repairs following an earthquake; <br />• Welded steel moment frames (WSMF) that constitute the primary seismic force -resisting <br />system for the building, and the structure was designed to code requirements preceding <br />those of the 1997 edition of the Uniform Building Code, and the building site has experienced <br />an earthquake of sufficient magnitude and site peak ground motions that inspection is <br />required when any of the conditions of Section 3.2 of FEMA 352 indicate an investigation of <br />beam -column connections is warranted; <br />• Visible signs of distress or deterioration of structural or non-structural systems, e.g„ <br />excessively cracked and/or spelling concrete walls or foundations, wood dry rot, etc. <br />Documentation of the selected performance level evaluation will be retained by the preparer and shall be <br />available to the State upon request. <br />Name: <br />Firm: <br />Telephone No: <br />License No: <br />License Expiration Date: <br />Ryan Bishoff <br />Brandow & Johnston, Inc. <br />949-862-8510 <br />SE 6250 <br />06130117 <br />� 11 /14/2016 <br />Signature Date <br />Comment: <br />For a building not qualifying under the above criteria; an Independent Review must be performed. <br />RELPS Option 2 30 April 09 <br />
