EXHIBIT 2 <br />paaoFrCT <br />nP :r:�. COergiP <br />Supportive Services Policy and Procedures <br />Effective July 1, 2019 <br />Purpose: <br />To establish guidelines for the issuance of supportive services to youth enrolled in a <br />WIOA funded program and are actively engaged In WIOA Title I activities and/or post - <br />exit follow up services. <br />References: <br />WIOA section 3(59). <br />Procedures: <br />ELIGIBILITY: <br />1. To request and receive support services, a participant must be actively enrolled <br />in one (1) or more of the fourteen (14) required program elements, including <br />during follow-up services <br />2. Supportive services will be approved upon verification of enrollment and <br />participation in required program elements, when it is necessary for youth to <br />participate in required program elements, and when the youth is unable to <br />obtain supportive services though any other program or agency. <br />3. Provision of support services is not an entitlement and it will be determined <br />what supportive services are reasonably required for youth based on Individual <br />need, assessment results, and the Individual Service Plan (ISP). <br />AVAILABILITY OF FUNDS AND ASSESSMENT OF NEED: <br />1. All Support Services not offered through PK are processed on a reimbursement <br />basis. <br />Z. Support services are delivered contingent upon availability of funds. <br />3. initial assessment of need will be determined by the Case Manager. The need <br />for services will be recorded on the Individual Service Plan (ISP) and through on- <br />going assessment. <br />4. Case Manager is responsible for determining if transportation is a significant <br />barrier to participation in required program elements, and whether ornotthe <br />youth were unable to obtain services through another program or agency. <br />25A-168 <br />
