The target <br />population will be 25 low-income women who are victims of crime and who reside in the City of Santa Ana. Crimes <br />include but are not limited to — domestic violence; sexual assault, assault, sexual battery, rape and robbery. Using <br />a Trauma Recovery Center model, the project incorporates - (1). Outreach: Victims are referred by community - <br />based partners such as Olive Crest; Catholic Charities, Salvation Army, OC Legal Aid, St. Olaf Lutheran Church, lst <br />Lutheran Church, St. Ni Lutheran Church, City of Anaheim, City of Fullerton, The Angels Baseball Foundation, <br />Community Service Department, Manpower, Orange County One Stop Center and Families Forward. LSSSC's <br />programs and services are listed in Orange County 2-1-1 (2-1-1 OC) system. LSSSC receives referrals through 2-1- <br />10C. LSSSC will also be assisted by local Lutheran congregations to identify potential participants. (2). Screening <br />and Enrollment: Potential clients will be screened for eligibility and enrollment at the TRC strategically located in <br />Santa Ana. (3). Assessment: The project utilizes evidence -based practices to assess clients for the most critical <br />needs. These practices include DSM-5 (PTSD); MHSF-III (mental impairment), CAGE -AID (substance abuse disorder) <br />and VI-SPDAT (tests homeless medical vulnerability). (4). Self -Sufficiency Planning: Based on the assessment, an <br />individualized self-sufficiency plan (ISSP) will be developed to triage the unique needs of clients. The plan includes <br />milestones and timeline for the completion of milestones. (5). Therapy: If the results of the screening suggest that <br />clients may have a predisposition to mental impairment, substance abuse disorder and/or PTSD and other medical <br />problems, two approaches are taken — (a). Referrals will be made to third -party providers for follow-up screening, <br />diagnosis and treatment of substance abuse disorder or severe mental illness. This includes psychotherapy, mental <br />health treatment and substance abuse treatment. (b). The Licensed Clinical Social Worker/Therapist provides <br />cognitive behavioral therapy and mental health counseling onsite. Therapy is performed using trauma -informed <br />and gender -sensitive approaches. Within this context, program staff recognize that survivor's need to be <br />respected, informed, connected, and hopeful regarding their own recovery; recognize the interrelation between <br />trauma and symptoms of trauma such as substance abuse, eating disorders, depression, and anxiety and recognize <br />the need to work in a collaborative way with survivors, family and friends of the survivor, and other human <br />services agencies in a manner that will empower survivors and consumers. (6). Intensive Case Management: The <br />Case Manager will follow up on clients' progress through one-on-one case management sessions. Program staff <br />will conduct individual and group counseling and therapy. Project staff will corroborate with local domestic <br />violence shelters to refer clients, especially those who are fleeing domestic violence. (7). Independent Living Skills <br />EXHIBIT A <br />
