Mental <br />health services will include safety planning, assessment,and individual, group and family therapy. <br />Funding for this program will be used to pay for a portion of a therapist's salary (.30 FTE), the direct program <br />supervisor's salary (.05 FTE), and the program manager salary (.025 FTE). Additionally funds are requested for the <br />following expenses: mileage, program supplies, staff training and telephone. <br />EXHIBIT A <br />
