STATEMENT REGARDING COMMUNITY WORKFORCE AGREEMENT (CWA) <br />REQUIREMENTS <br />This is to certify that the undersigned CONTRACTOR, and subcontractors, have read and understood the <br />CWA entered into by and between the City of Santa Ana, the Los Angeles/Orange Counties Building and <br />Construction Trades Council, and the signatory Craft Councils and Local Unions, effective as of <br />September 1, 2017. The CWA is available at: <br />http://www.santa-ana.org/vwa/dociuntmnents/CWA.D <br />The undersigned CONTRACTOR hereby agrees to comply with all terms and conditions of the CWA, <br />and is capable of completing construction of the project continuously, and without interruptions or delays. <br />If awarded any work covered by the CWA, CONTRACTOR will also be required to sign the Letter of <br />Assent that appears as Attachment A to the CWA. <br />Public Works Construction Permit: The Contractor may be required to obtain a Public Works <br />Construction Permit if the CWA thresholds for a Task Order are met or exceeded as described in this <br />RFD's Section ME "Licenses & Permits." <br />The undersigned has reviewed and understands that a Public Works Construction Permit for CWA <br />administration fees and deposit that may be required as described in this RFP's Section IH.E "Licenses & <br />Permits." <br />Signed: <br />Title: � Fr" rel", <br />Firm: <br />Date: /� — / �' a 0 <br />City of Santa Ana RFP 20-106 <br />Page Al-36 <br />
