EXHIBIT A <br />SCOPE OF WORK / PRICING <br />Background: The City is partnering with Contractor to provide COVID-19 testing to residents <br />of the City of Santa Ana. Contractor shall provide Federal Drug Administration (FDA) <br />authorized testing to Public Health priority populations utilizing a volume expandable kiosk <br />model. Residents will not have out-of-pocket expenses, but will need to provide insurance or <br />identification to facilitate reimbursements by insurers and the Health Resources Service <br />Administration (HRSA) for uninsured people. <br />City Responsibilities: <br />A. The City will provide Contractor a non -reimbursable advance, which the <br />Contractor shall expend for qualified use including but not limited to staffing, physical plant <br />(kiosk), transportation testing media and collected specimen. City will secure and pay as needed for the space required for testing services and <br />safe queuing of cars. This includes staffing and volunteers as needed for site set <br />up/break down and security. <br />C. City will work with venues and municipalities as needed to ensure traffic flow to <br />mitigate impact on surrounding neighborhoods and city streets. This includes staffing, <br />volunteers, and law enforcement support needed for traffic control. <br />D. City will provide health education materials and onsite signage to support the event <br />and outreach and promotional materials for use by media and partners. Messaging and <br />outreach will be coordinated with Contractor and partnering cities and venues. <br />E. City will translate Contractor's collateral materials in Spanish and Vietnamese <br />provided in a Word file and with five days advance request. Additional languages may <br />be done but may require a longer turn -around time, <br />F. To the extent resources allow, City will attempt to provide support with PPE in the <br />even the Contractor is unable to procure supplies in a timely manner through <br />commercial suppliers. <br />G. City will manage media inquiries, in conjunction with venue/City and Contractor's <br />communications teams. <br />19 <br />
