EXHIBIT A SCOPE OF WORK / PRICING Background: The City is partnering with Contractor to provide COVID-19 testing to residents of the City of Santa Ana. Contractor shall provide Federal Drug Administration (FDA) authorized testing to Public Health priority populations utilizing a volume expandable kiosk model. Residents will not have out-of-pocket expenses, but will need to provide insurance or identification to facilitate reimbursements by insurers and the Health Resources Service Administration (HRSA) for uninsured people. City Responsibilities: A. The City will provide Contractor a non -reimbursable advance, which the Contractor shall expend for qualified use including but not limited to staffing, physical plant (kiosk), transportation testing media and collected specimen. At the City's sole discretion, the advance may be increased or decreased if the anticipated daily volume is expected to increase or decrease based upon demand, changes to the service model, or other factors. City will secure and pay as needed for the space required for testing services and safe queuing of cars. This includes staffing and volunteers as needed for site set up/break down and security. C. City will work with venues and municipalities as needed to ensure traffic flow to mitigate impact on surrounding neighborhoods and city streets. This includes staffing, volunteers, and law enforcement support needed for traffic control. D. City will provide health education materials and onsite signage to support the event and outreach and promotional materials for use by media and partners. Messaging and outreach will be coordinated with Contractor and partnering cities and venues. E. City will translate Contractor's collateral materials in Spanish and Vietnamese provided in a Word file and with five days advance request. Additional languages may be done but may require a longer turn -around time, F. To the extent resources allow, City will attempt to provide support with PPE in the even the Contractor is unable to procure supplies in a timely manner through commercial suppliers. G. City will manage media inquiries, in conjunction with venue/City and Contractor's communications teams. 19
