ATTACHMENT 6 <br />COST/BID PROPOSAL FORM - RFP No. <br />NOTE 1: Submit and label a separate COST/BID Proposal Form for each proposed source of <br />revenue bid under Section III [Scope of Services and Time of Performance], subsection <br />(A) [Sources of Revenue] and/or for each desired response item bid - numbered (1) <br />through (5) - as described in Section I [Introduction], subsection (A) [Purpose]. <br />In compliance with the Agreement Documents, the undersigned hereby proposes to furnish all <br />required labor, materials, supervision, transportation, equipment, services, taxes and <br />incidentals required for (label) <br />The Work is to be done in strict conformity with the Agreement Documents now on file in the <br />Finance & Management Services Agency, Treasury & Customer Service Division, for the <br />following sum: <br />Property Tax Revenue Auditing, Recovery, Reporting, Analysis, and Legislative/State <br />Agency Liaison and Implementation Monitoring Services - Cost/Bid Proposal <br />All Fiscal Years below must be bid in order for the quote to be considered responsive except <br />where bidder specifies a future commencement date of performance. <br />Fiscal Year <br />Option 1 <br />Fixed Flat <br />Fee <br />Option 2 <br />Percentage of <br />Revenue <br />Source <br />Option 3 <br />Percentage of <br />Recovered <br />Revenues <br />Option 4 <br />Time and <br />Expenses <br />1. <br />FY 2020-2021 <br />2. <br />FY 2021-2022 <br />3. <br />FY 2022-2023 <br />4. <br />FY 2023-2024 <br />5. <br />FY 2024-2025 <br />Total Cost: <br />NU Lt z: At t•roposer s discretion trom 1 to 4 Options may be bid in alternative or in combination <br />to one another. <br />PROPOSER'S NAME: <br />If awarded the Agreement, the undersigned agrees to sign said Agreement and furnish the <br />necessary surety bonds and insurance certificates within thirty (30) days after receipt of the <br />notice of award of Agreement, and to begin work when directed by the City. <br />It is understood that this Cost/Bid Proposal is based upon completion of Work commencing on <br />the date set forth in the Agreement issued by the City to the Professional <br />Consultant/Vendor. <br />In determining the amount bid by each bidder, the City shall disregard mathematical errors in <br />addition, subtraction, multiplication, and division that appear obvious on the face of the <br />Proposal. When such a mathematical error appears on the face of the Proposal, the City shall <br />have the right to correct such error and to compute the total amount bid by said bidder on <br />the basis of the corrected figure or figures. <br />When an item price is required to be set forth in the Proposal, and the total for the item set forth <br />separately does not agree with a figure which is derived by multiplying the item price times the <br />City of Santa Ana — Revenue Auditing, Recovery, Reporting, Analysis, and Legislative/State Agency <br />Liaison and Implementation Monitoring Services <br />Request for Proposals No. 20-126 Page 23 <br />October 1, 2020 <br />
