Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the <br />funded ro ram, named above, during the 12-month contract period, <br />17 Persons <br />11, Number of unduplicated Santa Ana residents expected to be served by the funded program during the 12-month <br />contract period, <br />17 Persons <br />Program and Funding description <br />111. Description of Work» In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print, <br />Families Forward will provide Motel Vouchers to 4.5 homeless families - approximately 17 Indlvid uals - with <br />ESG-CV funds during the time between the closure of the Link Shelter and the opening of the new Carnegie <br />Shelter, CDBG-CV funds will be used to provide those families with Case Management Services since they will <br />not have access to these types of services due to the closure of the shelter, Supportive services like case <br />management will be offered as long as families are staying at the motel Keeping In mind the current funding <br />amount, we anticipate each family will be able to stay in a rnotel up to 2 months.
