City of Santa Ana <br />RFP; Homeless Navigation Center <br />PROJECT OUTCOMES <br />The performance outcomes for this site will be measured by the table below as well as <br />thresholds established in the annual Orange County Emergency Shelter Project Performance <br />Report, as a guideline. <br />Additionally, the Respondent and all partner organizations will be responsible for complying with <br />standards for Homeless Assistance, Homeless Management Information System (HMIS), and <br />CES. <br />All Navigation Center activities will be documented in monthly, quarterly and year-end reports in <br />a format determined by the City of Santa Ana.
