City of Santa Ana <br />RFP: Homeless Services - Street Outreach and Engagement <br />EXECUTIVE SUMMARY <br />The City of Santa Ana's Community Development Agency (CDA) is seeking proposals from <br />eligible respondents to enter into a public -private partnership to provide street outreach and <br />engagement to homeless neighbors in the city. The term of the agreement will be for two years <br />with two, one-year extensions. <br />Street outreach is service delivery for the specific purpose of reaching out to unsheltered <br />homeless neighbors; connecting them with emergency shelter, housing, or critical services; <br />and providing urgent, non -facility -based care. These activities are intended to help homeless <br />neighbors to begin the process of obtaining appropriate supportive services, including <br />assistance in obtaining permanent housing, medical health treatment, mental health treatment, <br />counseling supervision and other services essential for achieving independent living. <br />Engagement services are activities to locate, identify, and build relationships with unsheltered <br />homeless people to offer immediate support, intervention, and connections with homeless <br />assistance programs and/or mainstream social services and other housing programs. Specific <br />activities may include: <br />• Conducting an initial assessment of client needs and eligibility; <br />• Providing informal crisis counseling; <br />• Addressing urgent physical needs, such as providing meals, blankets, clothes, or <br />toiletries; <br />• Actively connecting and providing information and referrals to needed services; <br />• Making formal referrals/transport into shelter, including those in the Orange County <br />CoC.. <br />Funding will be provided by the City of Santa Ana through various funding sources outlined <br />within this RFP. Only non-profit organizations currently providing services to individuals <br />experiencing homelessness and which meet the minimum submission requirements will be <br />considered. <br />Santa Ana expects respondents to be fully ready to provide outreach and engagement services <br />effective June 15, 2021. <br />4 <br />
