City of Santa Ana <br />RFP Homeless Services - Street Outreach and Engagement <br />Program Components <br />• Provide placement in emergency shelter, transitional housing <br />permanent housing or family reunification as a primary goal. <br />• Case Management <br />• Outreach and Engagement Specialists <br />• Client support services <br />• Transportation/Shuttle Services <br />• HMIS Participation and Documentation <br />• Participation in the Coordinated Entry System (CES) <br />• Coordination with the City of Santa Ana Housing Authority, <br />referrals to County, State and Federal programs, as well as <br />nonprofit and social service agencies, as appropriate <br />• Follow best practice models such as progressive engagement, <br />motivational interviewing, and other strength based practices <br />• Eligibility screening and referral processes to shelters, <br />recuperative care programs, drug rehabilitation centers, crisis <br />stabilization units, mental health clinics, medical health facilities <br />and additional appropriate County Behavioral Health, Health <br />Care and Social Services programs that are made available froi <br />public and private sectors <br />General Standards • Adequate staffing with appropriate on -going staff training <br />• Designated point -of -contact who is available at all times to <br />address issues that may arise and willingness to coordinate <br />security issues with the Santa Ana Police Department <br />• Means for clients to provide input into programs <br />• Data entry, analysis and reporting in the HMIS. <br />• Monthly reports to the City. <br />• Respondent will maintain all appropriate documentation. <br />The performance outcomes for this project will be measured by the table below. Additionally, <br />the Respondent and all partner organizations will be responsible for complying with standards <br />for Homeless Assistance, Homeless Management Information System (HMIS), and CES. <br />All outreach and engagement activities will be documented in monthly, quarterly and year-end <br />reports in a format determined by the City of Santa Ana.
