INSURANCE ON FlLE <br />WORK MAY PROCEED <br />aylol / z�ZZ,e..� <br />FIRST AMENDMENT TO <br />AGREEMENT WITH HALCYON BEHAVIORAL, LLC. <br />N-2021- Zol-ay <br />THIS FIRST AMENDMENT TO AGREEMENT is entered into this 161° day of November 2021, by <br />and between Halcyon Behavioral, LLC, a California limited liability company doing business as <br />Halcyon Behavioral Administrators ("Consultant"), and the City of Santa Ana, a charter city <br />and municipal corporation duly organized and existing under the Constitution and laws of the State of <br />California ("City"). Consultant and City are also referred to herein as "Parties" or individually as a <br />"Party.,, <br />na;m�e�)(1)S,`^RECITALS <br />A. On September 21, 2021, the City entered into Agreement No. N-2021-201 ("Agreement") <br />with Halcyon Behavioral, LLC, a California limited liability company doing business as <br />Halcyon Behavioral Administrators, to provide employee wellness program services, and <br />related matters, for the tern of three years. <br />B. The parties desire to amend the scope of services provided by Consultant and the insurance <br />requirements under the Agreement. <br />THE PARTIES THEREFORE AGREE: <br />Section 1, SCOPE OF SERVICES, shall be amended to exclude Biometric Screening <br />Coordination (Exhibit A to the Agreement, Item 2). <br />2. Section 6, INSURANCE, shall be amended to require Professional Liability Insurance with <br />a limit of no less than $1,000,000 per occurrence or claim, $2,000,000 aggregate. <br />3.
