Exhibit 1 <br /> APPENDIX <br /> ATTACHMENT 3-6: QUALIFICATIONS <br /> CITY OF SANTA ANA <br /> REQUEST FOR PROPOSALS <br /> RFP NO.: 17-031 <br /> Include this page as a cover for your response to this section. <br /> -(Max 3 pages plus resumes)Proposal shall include a profile of the firm's experience. Include resumes of project <br /> team/sub-contractors that outline their technical and design experience. This should include the project <br /> manager/principal agent, associates in charge when project manager/principal agent is unavailable,key personnel, <br /> firm size, and an organization chart identifying only those who will perform work for the proposed project and <br /> the percentage of each individual's time devoted to this project. The project manager/principal agent shall be the <br /> primary contact person to represent your firm. <br /> Proposal shall include the number of proposed full-time equivalent personnel (FTE's) by job class/function. <br /> Proposal shall also include a listing of equipment and materials that will be used to complete the scope of work. <br /> -(Max 4 pages) Proposal shall include an outline, which demonstrates PROPOSER understands of the scope of <br /> work. This outline should include anticipated approach, tasks necessary for successful completion, and <br /> suggestions or special concerns for which the City should be made aware. <br /> -(Max 5 Pages) Proposal shall include a Scope of Services and Schedule which details the work phases to be <br /> completed, the tasks to be accomplished, the deliverables to be provided, and the schedule/timeline to complete <br /> the project,based upon the requested Scope of Work, detailed in Attachment 1,which is included in the Appendix <br /> of this RFP. <br /> -(Max 1 Page) As this project will require adherence to a strict timeline, provide information on your current <br /> workload and how you would accommodate this project. <br /> City Council 25 — 59 5/3/2022 <br />
