Participants will receive the support <br />and resources needed to remain in medical care, maintain their health, and achieve viral suppression. <br />America On Track <br />Brighter Futures <br />for Children of <br />Prisoners <br />$49,268 <br />Children of prisoners deserve the chance to discover a brighter future, which is why America On Track <br />provides them with trained, caring mentors, STEM workshops, a “College is For Me Too!” camp, free books, <br />and special outings, as well as nutrition seminars and wraparound services for the families. <br />Boys and Girls Club of <br />Santa Ana College Bound $27,090 <br />College Bound provides at-risk Santa Ana teens with academic instruction and mentors to help graduate <br />high school on time, prepare for secondary education, technical school, or military and set career goals; this <br />also prevents teens from engaging in activities that could otherwise derail their path. <br />Casa de la Familia <br />Santa Ana Family <br />Justice Center <br />Full-Time Victim <br />Advocate <br />$27,090 <br />SAFHC Full-Time Victim Advocate will offer more comprehensive services to victims than before allowing <br />victims to reach necessary resources previously almost inaccessible. Providing a full-time advocate helps <br />victims manage all services needed through one person, easing the access. <br />Community Action <br />Partnership of Orange <br />County <br />Economic <br />Empowerment – <br />Your Money, Your <br />Goals <br />$27,090 <br />Economic empowerment services for low-income adults and youth in and around Santa Ana; workshops <br />include Your Money Your Goals curriculum, which covers banking, spending plans, and money <br />management by providing tools for participants to develop habits to increase their financial wellbeing. <br />Community Health <br />Initiative Orange County <br />Community Health <br />Access Program $27,090 <br />CHIOC helps City of Santa Ana residents to access health and social service programs by providing <br />outreach, education, enrollment, and case management services to vulnerable populations. We strive to <br />ensure that clients acquire, retain, and utilize community resources effectively. <br />Community Legal Aid <br />SoCal <br />Santa Ana <br />Domestic Violence <br />Prevention Project <br />$27,090 <br />Community Legal Aid SoCal (previously known as The Legal Aid Society of Orange County) will provide <br />free, holistic legal assistance to Santa Ana survivors of domestic violence to help them escape abuse and <br />break the cycle of violence. Priority is given to service in the areas of family law, immigration, and healthcare <br />advocacy. <br />Delhi Center Family Economic <br />Success Initiative $58,298 <br />A collaborative case management strategy to help low-income families fill vocational, educational, and <br />financial service gaps, so they can make better decisions to earn more, keep more of what they earn, and <br />move up the economic ladder. <br />Delhi Center <br />Teens Engaged in <br />Learning and <br />Leadership <br />$36,120 <br />A leadership development program that trains teens to take an active role in designing and implementing <br />educational and team building activities in their community that teach younger children about gang <br />prevention, problem solving, conflict resolution, avoiding risk and making safe choices. <br />Girl’s Inc. of Orange <br />County Literacy Lab $27,090 <br />Literacy Lab ensures that 120 elementary school girls per year in Santa Ana reach grade level reading by <br />fourth grade. Small group reading with mentors and educators, engaging activities, a summer camp, and <br />family literacy nights build a love of reading and a strong educational foundation. <br />Human Options Assisting Domestic <br />Violence Victims $27,090 <br />The project will provide intensive case management services to Santa Ana residents who self-report <br />domestic violence or who are referred by the Domestic Violence Response Team. Case management <br />services will include safety planning, assessment of needs and linkages to needed resources. <br />EXHIBIT 3
