45 <br />ARTICLE XII <br /> <br />12.0 EMPLOYEE INSURANCE <br /> <br />12.1 The City will contribute toward the payment of premiums for health, dental and long-term <br />disability insurance plans administered by the Association for the benefit of the employees <br />represented by the Association as follows: <br /> <br />A. Medical Insurance. The City will contribute $1,575.00 per month to the Santa Ana <br />Police Officers Association Medical Insurance Trust Fund for each affected <br />employee enrolled in medical insurance plans provided by the Association for its <br />bargaining unit members and their eligible dependents. Any contributions <br />necessary to maintain benefits under the plans in excess of the aforementioned <br />amount per month shall be borne entirely by the Association and/or the enrollee. <br />Members of the bargaining unit who are not Association members and therefore <br />not eligible to purchase insurance through the Trust will receive from the City up <br />to $1,575.00 per month to be used only for the purchase of medical insurance for <br />themselves and eligible dependents. <br /> <br />The Association will provide the City with proof that each of the employees in the <br />Association who have enrolled in insurance are receiving insurance. The <br />Association will also provide the City (once each quarter) with a list of the group <br />insurance plan and rates for medical, dental and Long Term Disability. <br /> <br />Effective January 1, 2024, the City will begin to administer medical and dental <br />insurance for active employees. The City will contribute the following amounts for <br />each employee: <br /> <br />Employee Only - $804.00 <br />Employee + 1 - $1,457.00 <br />Employee + family - $1,850.00 <br />These amounts are inclusive of the CalPERS statutory minimum ($151 for 2023 <br />but not yet known for 2024). <br /> <br />Dental – Cost of coverage up to $130. <br /> <br />Long-Term Disability Insurance – Effective January 1, 2024, the City will begin <br />to administer Long-Term Disability insurance. The City will agree to pay 100% of <br />the premium for a long-term disability policy with a 60-day elimination period up <br />to a maximum of $10,000 per month. <br /> <br /> <br />B. Life Insurance. The City shall provide term life insurance coverage for each <br />affected employee in the amount of thirty thousand dollars ($30,000), plus twenty <br />thousand dollars ($20,000) accidental death and dismemberment (AD + D) <br />coverage. <br />
