ARTICLE XII <br />12.0 EMPLOYEE INSURANCE <br />12.1 The City will contribute toward the payment of premiums for health, dental, and long-term <br />disability insurance plans administered by the Association for the benefit of the employees <br />represented by the Association as follows: <br />A. Medical Insurance. The City will contribute one thousand five hundred seventy- <br />five dollars ($1,575.00) per month to the Santa Ana Police Officers Association <br />Medical Insurance Trust Fund for each affected employee enrolled in medical <br />insurance plans provided by the Association for its bargaining unit members and <br />their eligible dependents. Any contributions necessary to maintain benefits under <br />the plans in excess of the aforementioned amount per month shall be borne entirely <br />by the Association and/or the enrollee. Members of the bargaining unit who are <br />not Association members and therefore not eligible to purchase insurance through <br />the Trust will receive from the City up to one thousand five hundred seventy-five <br />dollars ($1,575.00) per month to be used only for the purchase of medical insurance <br />for themselves and eligible dependents. <br />The Association will provide the City with proof that each of the employees in the <br />Association who have enrolled in insurance are receiving insurance. The <br />Association will also provide the City (once each quarter) with a list of the group <br />insurance plan and rates for medical, dental and Long Term Disability. <br />The Association shall provide and administer health benefit plans for employees in <br />accordance with the terms and conditions of the Health Plan Agreement attached <br />as Exhibit C to this MOU. <br />The parties agree to reopen this 2024-2027 MOU to consider the City's health <br />insurance contribution after receiving the results of the 2024 Association Medical <br />Insurance Trust Fund audit. <br />B. Life Insurance. The City shall provide term life insurance coverage for each <br />affected employee in the amount of thirty thousand dollars ($30,000), plus thirty <br />thousand dollars ($30,000) accidental death and dismemberment (AD + D) <br />coverage, subject to carrier contract, terms, and conditions. <br />12.2 Retiree Health Contribution. <br />All employees covered by this MOU will contribute two percent (2%) of their regular rate <br />of pay through payroll deduction to a fund maintained by the Santa Ana Police Officers <br />Association for the purpose of providing retiree health insurance premium reduction <br />assistance. <br />On October 1 of each year, the City shall contribute an amount equal to two percent (2%) <br />of each employee's regular rate of pay of each active employee who is covered by this <br />MOU as of October 1 of each year to a fund maintained by the Santa Ana Police Officers <br />47 <br />
