EXHIBIT B - COSTS/RATES <br />Core Wellness Plus Program: <br />The cost of the Core Wellness Plus Program to the City for all CIs will be $1.05 per employee <br />per month (PEPM). This PEPM pricing for the Core Wellness Plus Program will be effective for <br />the duration of the agreement and will only increase if the City requests additional services. <br />Billing for the service will occur the first week of the month after the delivery of services during <br />the previous month, based upon the number of CIs identified by the City multiplied by the <br />PEPM. <br />
