Participants will receive the support and resources needed to remain in medical <br />care, maintain their health, and achieve viral suppression. <br />According to the Orange County Health Care Agency (OCHCA), by the end of 2020, 7,466 persons were living with HIV <br />disease (PLWHD) in the region-6,548 who were aware and 918 who were unaware of their HIV status. In 2020, 265 <br />persons were newly diagnosed with HIV in the county while 56 persons were concurrently diagnosed with AIDS — <br />indicating that the individual was living with HIV but was unaware of their status for a significant amount of time. The City <br />of Santa Ana has the highest rate of newly diagnosed HIV cases in the County. Through RHC's wraparound HIV care <br />services, individuals living with HIV receive the support and resources needed to stay in medical care, maintain their <br />health, and achieve viral suppression (low levels of HIV in the body —less than 200 copies per milliliter of blood). People <br />whose HIV viral load is stably suppressed cannot sexually transmit the virus. As a result, the number of new HIV infections <br />in our community is greatly reduced and the community saves thousands of dollars each year by reducing the burden on <br />safety net services. <br />RHC provides a wide array of programs and services to people living with HIV in Santa Ana. Services are provided at <br />our offices in Irvine and Santa Ana, CA and at a variety of locations throughout Orange County, including <br />community buildings, skilled nursing facilities, hospitals, and in homes of clients who have transportation barriers <br />or are too ill to commute. These services include: <br />1►. HIV Testing: Testing is provided on a walk-in basis at our offices in Irvine and Santa Ana five days per week and <br />are completely free and confidential. To reach a larger portion of the community, RHC also provides weekly testing <br />services and outreach at several local bars frequented by our priority population, at community events, CBO sites <br />and homeless shelters. <br />2). HIV Medical Case Management: This comprehensive care program addresses the individual client, breaks down <br />barriers to care, and facilitates autonomy and movement towards self-sufficiency. Case managers link clients to the <br />array of free services provided by RHC and assist in securing public and private benefits available in the wider <br />community.
