EXHIBIT A <br /> <br />ASSURANCES, GENERAL PROVISIONS AND STANDARDS OF CONDUCT <br />ASSURANCES SPECI~IC TO JOBS FOR CALIFORNIA GRADUATES AND THE JAG MODEL <br /> <br /> Subgrantee agrees to the following: <br /> 1. Enroll 6 WIA-eligible youth into JTA according to the Local Plan of Service(Exhibit B).. <br /> 2. Follow-up on previous year participants and record into JTA <br />3. Meet performance goals or if goals cannot be met, explain in the Plan of Service how Subgrantee will <br /> address the goals: <br /> a) Youth 19-21: <br /> i) Entered employment: 56% <br /> ii) Retention Rate: 72% <br /> iii) Average Earnings Changes: $2,600 <br /> iv) Credential Rate: 42% <br />4. Attend ALL training related to WIA application completion, eligibility documentation and other <br /> meetings regarding administration of funds. <br />5. Address how alll0 components of the JAG Model will be implemented locally in the Local Plan of <br /> Service, as part of this contract (Exhibit B). The Plan should Include: <br /> a) A Table of Organization showing the relationship between the local board and local staff, <br /> including an explanation of the supervisory structure. <br /> b) Provisions to enlist and gain local public and private sector participation and support. <br /> c) Any requests for variance from the JAG Model. Explain and justify. <br />6. Staff Development will be an integral part of the Subgrantee's program operation. <br /> a) All new staff will receive comprehensive training on the JAG Model and the JAG NDMS by a <br /> JAG-approved trainer. JAG-approved trainers must participate in annual re-certification <br /> workshops. <br /> b) Subgrantee may participate in state and national meetings, in-service workshops and JAG NTS <br /> and JCG State training. Travel and per diem costs will be defrayed by the local JCG affiliate. <br />7. JCG students and local affiliate staff will be permitted and encouraged to participate in statewide JCG <br /> activities and conferences, including the regional and statewide Career Development Conference and <br /> the State Leadership Conference. <br />8. JAG data will be submitted to JCG, Inc. electronically using JAG's Data Management System. <br /> a) Data will be electronically transmitted monthly to the JCG State Office by no later than the 8th of <br /> each month. <br /> b) Completed Student Satisfaction Surveys will be due to the JCG State Office by May 30. <br />9. WIA applications and other paperwork will be submitted to the JCG Southern Regional Coordinator <br /> completely and in a timely manner, pursuant to procedures and processes developed. Subcontractor <br /> will be responsible for accurate and complete applications and required documentation. <br />~10. Allow JAG National Accreditation staff to conduct a site visit, interview staff and students, and <br /> observe training. <br />~l ~l. Monthly expenditure reports will be due on the 8th of each month and must include a detailed report of <br /> actual expenditures. <br />~l 2. Monthly narrative report will be submitted by the 10th of each month, to include progress in achieving <br /> grant objectives, problems associated with grant implementation, notable accomplishments, and <br /> training or technical assistance needs. <br /> ~13. An annual audit of the Subgrantee will be completed and a copy of the audit will be submitted to JCG. <br /> ~l 4. All PR/Marketing materials designed/developed related to this grant must include the phrase: "An <br /> affiliate of Jobs for California Graduates and Jobs for America's Graduates." <br /> <br />2 <br /> <br /> <br />
