THIS ENDORSEMENT CHANGES THE POLICY. PLEASE READ IT CAREFULLY. <br />NOTICE OF CANCELLATION TO CERTIFICATE HOLDER(S) <br />This policy is subject to the following additional Conditions: <br />A.IfthispolicyiscancelledbytheCompany,otherthanfornon-paymentofpremium,noticeofsuchcancellationwillbe <br />providedatleastthirty(30)daysinadvanceofthecancellationeffectivedatetothecertificateholder(s)withmailing <br />addresses on file with the agent of record or the Company. <br />B.Ifthispolicyiscancelledbythecompanyfornon-paymentofpremium,orbytheinsured,noticeofsuchcancellation <br />willbeprovidedwithinten(10)daysofthecancellationeffectivedatetothecertificateholder(s)withmailing <br />addresses on file with the agent of record or the Company. <br />Ifnoticeismailed,proofofmailingtothelastknownmailingaddressofthecertificateholder(s)onfilewiththeagentof <br />record or the Company will be sufficient proof of notice. <br />Anynotificationrightsprovidedbythisendorsementapplyonlytoactivecertificateholder(s)whowereissuedacertificate <br />of insurance applicable to this policy’s term. <br />Failuretoprovidesuchnoticetothecertificateholder(s)willnotamendorextendthedatethecancellationbecomes <br />effective,norwillitnegatecancellationofthepolicy.Failuretosendnoticeshallimposenoliabilityofanykinduponthe <br />Company or its agents or representatives. <br />CzDzouijbNpsbbu:;44bn-Pdu3:-3135 <br />Form SL 90 13 10 18Page1 of1 <br />© 2018, The Hartford <br />(May include copyrighted material of Insurance Services Office, Inc., with its permission) <br />
