Professional Liability Insurance Policy The Named Insured may purchase an Optional Extended Reporting Period of up to thirty six (36) months beginning on the effective date of termination of this Policy for an additional premium, provided that: 1. The Named Insured makes a request, in writing, for such Optional Extended Reporting Period, which is received by the Company within thirty (30) days of the effective date of termination of this Policy; 2. The additional premium for an Optional Extended Reporting Period will be calculated by the Company and shall not exceed: 100% of the expiring annual premium for a one (1) year period; 175% of the expiring annual premium for a two (2) year period; and 250% of the expiring annual premium for a three (3) year period. 3. The Named Insured shall pay the Company the additional premium within sixty (60) days of the effective date of termination of this Policy, and once paid, such premium shall be fully earned. The Optional Extended Reporting Period shall apply as follows: A Claim first made against the Insured during the Policy Period and first reported to the Company, in writing, during the Optional Extended Reporting Period will be deemed to have been made and reported on the last day of the Policy Period, provided that such Claim is otherwise covered under this Policy. All such Claims shall arise out of a Wrongful Act in the performance or non-performance of Professional Services that was committed on or after the Retroactive Date and before the end of the Policy Period. 2. A Claim first made against the Insured and reported to the Company, in writing, during the Optional Extended Reporting Period will be deemed to have been made and reported on the last day of the Policy Period. 3.
