NAME: CITY OF SANTA ANA PUgUC WORK kNCY <br />PERMT: 1296-NLF-0213 <br />DATE: MAY 29,1996 <br />Permittee's Contractor will be required to pay a set fee of $210.00 to cover Caltrans, inspection <br />costs. <br />In addition to the attached Permit General Provisions, Form TR-0045 (Rev. 9/95), the following <br />Permit Special Provisions are applicable: <br />1- Permittee shall contact California Highway Patrol Area #675 Commander at 714/547-8311 <br />and OCTA DETOUR COORDINATOR (714/265-4330) at least 48 hours prior to <br />implementing traffic control which requires lane closures. <br />2- Permittee shall notify UNDERGROUND SERVICE ALERT at 1-8W/422-4133 a minimum of <br />two working days prior to the start of any excavation authorized by this Permit. <br />3- Orange vests and hard hats shall be worn at all times while working within State right- <br />of-way. <br />4- All traffic control within State right of way shall be provided, installed and maintained <br />by Permittee in accordance with State Standards and subject to the approval of the State <br />Permit Inspector. All work which requires traffic control shall only be performed on <br />weekdays between 9:30 AM and 2:30 PM. <br />5- All traffic control which requires lane closures shall be in compliance with the appropriate <br />STANDARD PLANS traffic control system. Where required by the plan, the .use of a <br />flashing arrowboard is MANDATORY. <br />6- All work within 6 feet of a traffic lane shall require closure of that lane to public traffic. <br />7- The closure of freeway lanes or ramp lanes is not authorized by this permit. <br />8- Personal vehicles of the Contractor's employees shall not be parked on paved shoulders <br />or traveled way within the limits of this work. <br />9- In the event of any discrepancy between Permittee's Permit Plans and these Permit <br />Special Provisions, these Special Provisions shall prevail. <br />10- All open excavations within State right of way shall be either backfilled by the end of <br />each working day, or otherwise secured to protect the public. <br />11- Pennittee shall immediately repair/replace all existing State facilities which are damaged <br />during the performance of his permitted work, to the satisfaction of the State Permit <br />Inspector and at no cost to the State. <br />12- The full width of traveled way shall be open for use by public traffic on Saturdays, <br />Sundays and designated legal holidays, after 2:30 PM on Fridays and on the day <br />preceding designated legal holidays, and when construction operations are not actively in <br />progress. <br />99- Immediately following completion of the work permitted herein, Permittee shall fill out <br />and mail the attached postcard to initiate final permit processing. <br />Page 2 of 2 <br />
