EXHIBIT B <br />STATEMENT OF WORK <br />Scope of Work. City hereby engages contractor as an independent contractor to <br />perform the following described work, and contractor hereby agrees to perform said <br />work upon the terms and conditions here -in -after set forth. <br />A. Specifically, contractor shall perform the following services: <br />Contractor will provide staff training and job search Internet workshops for <br />eligible Dislocated Worker participants. Such workshops shall occur in <br />accordance with the schedule, and curriculum attached hereto as the program <br />narrative (Exhibit A). Neither the seminar schedule nor curriculum may be <br />altered in any way during the term of this agreement, without prior written <br />approval given to contractor by City. <br />2. Full completion of the workshops shall be defined as and determined by <br />individual participant attainment of the learning objectives and demonstrated <br />achievement of the standards for evaluation listed in the curriculum verified <br />and approved by the City. <br />3. Contractor will provide each participant who attains completion of the <br />workshop with a certificate of completion. Such certificate shall serve as <br />contractor's assurance to the City that the recipient of said certificate has <br />satisfied the conditions of full completion as set forth in conditions identified in <br />paragraph 2 above. <br />4. Contractor will maintain attendance records for all participants. <br />5. Contractor shall, upon request by City or in accordance with schedules and/or <br />procedures stipulated by City staff at the outset of the workshops, transmit to <br />City reports and/or forms containing all appropriate data and any other <br />pertinent information available on each participant and program operations in <br />general. <br />6. Contractor will follow and abide by the proposal submitted and approved by <br />the Santa Ana Workforce Investment Board. Any changes to the proposal <br />must be submitted in writing to the CITY for consideration. <br />
