V. GOALS AND OBJECTIVES <br />METHODOLOGY INCARRYIN THEM OUT <br />Goal: To assist families, the frail elderly and individuals experiencing challenges to <br />independent living to maintain or secure safe, decent and affordable housing and <br />achieve self-sufficiency. <br />Objective(s) <br />1, Using FY 1999/2000 service levels to Santa Ana residents, to increase the number of low- <br />income single parents, families, seniors and disadvantaged singles residing in Santa Ana who <br />are provided assistance to avoid homelessness or institutionalization and achieve self- <br />sufficiency by 7% (from 3,239 unduplicated residents to 3,466 unduplicated residents). <br />Task/Action <br />(A) Secure Santa Ana ESG funding to support additional rental and utility assistance <br />payments for residents of Santa Ana - prior to programyear. <br />(B) Inform agencies collaborating with LSS/OC EA of additional rental and utility assistance <br />available to assist Santa Ana residents - upon notification of award. <br />(C) Obtain support for LSS/OC EA Santa Ana ESG program operational costs - prior to <br />program ye <br />(D) Provide rental/utility assistance and other forms of support leading to self-sufficiency to <br />660 Santa Ana households through: client intake, assessment and case management <br />services; provision of needed collateral support for food, clothing, infant products, ESL <br />courses, help in obtaining personal documentation, medical prescription payment <br />assistance, information and referral services, follow-up with referral agencies and <br />documentation regarding client progress - 1" through 4'h quarters. <br />2. To provide 50 monthly bus passes and 50 units of volunteer transportation and escort <br />services to residents of Santa Ana to: maintain, prepare for or obtain employment, attend <br />medical and other appointments for mental or physical health -related purposes, seek <br />assistance from social services providers or other trips required to maintain or achieve self- <br />sufficiency. <br />Task/Action <br />(A) Obtain funds and establish eligibility and intake procedures for providing 50 monthly <br />bus passes to those requiring assistance with local transportation - prior to r am year. <br />(B) Increase awareness among LSS/OC volunteers providing transportation and escort <br />assistance of the LSS/OC Santa Ana ESG program through announcements at churches, <br />printed materials and regular communication with current Caring Neighbors Shared <br />Housing and EA volunteers - I' and 2nd quarters. <br />(C) Inform Santa Ana residents of new transportation services provided by LSS/OC through <br />outreach efforts, to shelters, senior centers, agencies and programs serving single parent <br />families, seniors and those with physical and mental disabilities - 1 S` and 2nd quarters. <br />(D) Provide agencies serving eligible clients with transportation vouchers to be given to <br />clients for travel to and from the EA center in Garden Grove for the purposes of program <br />intake - I" through 41 quarter. <br />3.
