<br />to provide SERVICE and understand the City's insurance requirements. While I have the
<br />necessary insurance coverage plus the Additional Ins\lred Endorsement, as required, my
<br />insurance carrier, Zurich V,S. Insurance, will nol agree to the cross-outs in the
<br />cancellation clause (bottom right hand comer).

<br />that I will personally provide the City with the required 30 days' notice should my
<br />coverage be cancelled or materially reduced in amounts.
