EXHIBIT A <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Number of unduplicated Santa Ana residents to be served with grant funds during the 12- <br />month contract period = 30 PERSONS <br />H. Description of Work - in space below, describe the program to be funded during the 12-month contract <br />period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. <br />Services to be provided include community service information and referral, comprehensive in - <br />home assessment with a case manager, continual monitoring: weekly to bi-weekly monitoring by <br />telephone, bi-monthly to quarterly in -home visits, help arranging services, and in some cases full <br />payment of services/equipment for low income clients ineligible for other programs. <br />Program goal is to assist older and/or dependent adults prolong their wellness and independence <br />by assessing their needs then linking them with appropriate health and social services. <br />Client characteristics include any person aged 18 years or older living in Orange County with <br />any problem that puts that person at risk of being institutionalized. <br />Grant funds will be used to supplement the case manager that serves Santa Ana residents. <br />Schedule ofPerformance <br />Estimate the number of unduplicated Santa Ana residents to be served during the 12-month <br />contract period per quarter: <br />Quarter 1: July 1 — September 30 8 Persons <br />Quarter 2: October 1 — December 31 8 Persons <br />Quarter 3: January 1 — March 31 8 Persons <br />Quarter 4: April 1 — June 30 6 Persons <br />Total 30 Persons <br />Invoicing Schedule <br />Estimated the amount of grant funds to be requested during the 12-month contract period on a <br />quarterly basis: <br />Quarter 1: July 1 — September 30 $ 1,250 <br />Quarter 2: October I — December 31 $ 1,250 <br />Quarter 3: January 1 — March 31 $ 1,250 <br />Quarter 4: April 1 — June 30 $ 1,250 <br />Total $ 5,000 <br />Exhibit A <br />
