EXIIIBrr A <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Number of unduplicated Santa Ana residents to be served with grant funds during the 12 - <br />month contract period = 11-liPERSONS <br />II. Description of Work - in space below, describe the program to be funded during the 12 -month contract <br />period including, smites to be provided, program goals, client characteristics, k how grant funds will be utilized. <br />The Perinatal Nurse conducts our Health Care and Education Program and provides on-site perinatal <br />care and a wide range of health services to current Casa Teresa residents and their infants, many of <br />whom originate from the greater Santa Ana area. <br />Most of the women who enter the Casa Teresa Program are considered high risk due to poor family <br />relationships, homelessness, a history of smoking, alcohol or drug abuse, and poor nutrition due to <br />low income status. Many conte to Casa Teresa at the beginning of their second trimester and have <br />had no perinatal care. In addition, they may have had poor role models for learning proper parenting <br />skills, <br />It is with the nurse's valuable instruction that our mothers -to -be (both from the Parenting and <br />Adoption Programs) learn vital information about preventing birth defects and reducing low birth <br />weights. Her aim is not only to ensure healthy births, but to also assist the new mothers in acquiring <br />better parenting skills, which aids in the prevention of incidents of abuse and neglect. <br />Each year, approximately 70 mothers and their infants find shelter at Casa Teresa. In addition, <br />more than 400 women from the Orange and Santa Ana areas are supplied telephone referrals or <br />one-time counseling. <br />The Program Objectives and Methods Are To: <br />Reduce Pre -term Labor and Early Birth, <br />a Eliminate Post -Partum Infection. <br />a Reduce/Eliminate Post -Partum Depression. <br />a Educate the Women of the Value of Stroking and Drug Cessation During Pregnancy and <br />the Dangers of Second -Hand Smoke. <br />a Educate the women on the importance of a nutritional diet and healthy lifestyle. <br />a Increase the Number of Mothers Who Choose to Breastfeed. <br />a Teach Mothers to Recognize Health, Well Being and Illness in the Baby. <br />a Facilitate Healthy Bonding and Attachment Between Mother and Baby. <br />a Teach Basic Baby Care. <br />a Focus on Feeding and Nutrition. <br />a Promote Family Planning and Birth Control. <br />a Emphasize the Value of Immunization and the Value of Well -Baby Check Ups. <br />Exhibit A <br />
