<br />EXHIBIT A <br /> <br />SCOPE OF SERVICES <br /> <br />I. CLASS OBJECTIVE <br /> <br />A. CONSULTANT shall provide children's ballet dance instruction for children age 4 to 12. <br />CONSULTANT will offer professional level instruction. Traditional ballet terminology <br />and technique will be utilized. Sequential instruction will be utilized for optimum <br />instruction. <br /> <br />B. Consultant will confer with City staff to set class times and location, including days and <br />hours class will be taught, the location of said classes and the holidays which will be <br />observed. <br /> <br />C. CONSULTANT shall provide and be responsible for equipment, records, personnel and <br />clean up of facilities and material necessary to ensure the effectiveness of said <br />instruction. <br /> <br />II. CLASS SIZE REGISTRATION <br /> <br />A. The minimum number of students is 15 per class, the maximum is 20 students. <br /> <br />B. No registration shall be accepted after the second week of classes. <br /> <br />C. In the event the minimum number of enrollees is not realized by the second week of <br />classes, the class shall be canceled and Consultant will be under no obligation to provide <br />services, nor will she receive compensation. <br /> <br />III. COMPENSATION <br /> <br />A. The fee to participants shall be no more than $20.00 per participant, per month. No <br />refunds shall be made to participants, unless class is canceled as set forth in Section II, <br />c., above. <br /> <br />B. City shall register and collect said fees from each participant in the class during the <br />registration period. City shall pay Consultant seventy percent (70%) of the total fees <br />within fifteen (15) working days after completion of each monthly session. City shall <br />retain thirty percent (30%) offees collected. <br /> <br />C. Consultant agrees City is entitled to audit Consultant's records to ensure compliance with <br />this Agreement. <br />
