<br />. <br /> <br />8. Provider shall follow and abide by the proposal submitted and <br />approved by the Santa Ana Workforce Investment Board Youth <br />Council. Any changes to the proposal must be submitted in <br />writing to the Santa Ana WIB Youth Council for consideration. <br /> <br />9. Provider shall maintain daily, weekly, and monthly classroom <br />and internship training time and attendance records for all <br />participants. Copies of attendance records shall be placed in <br />participant's file upon completion of program. <br /> <br />10. Provider shall maintain individualized case management records <br />and shall make these records available for review and <br />inspection upon request by City staff. All such records shall <br />remain otherwise confidential and shall be kept in a form <br />approved by The Santa Ana WIB Youth Council. <br /> <br />11. Full completion of the program and its elements shall be defined <br />as and determined by individual participant attainment of the <br />learning objectives and demonstrated achievement of the <br />standards for evaluation listed in the training curriculum verified <br />and approved by the Santa Ana WIB Youth Council. <br /> <br />12. Provider shall provide each participant who attains full <br />completion of the program with a certificate of completion. <br />Such certificate shall serve as contractor's assurance to the <br />City that the recipient of said certificate has satisfied the <br />conditions of full completion as set forth in conditions identified <br />in the Work Plan. <br /> <br />13. Provider shall, upon request by WIB staff or in accordance with <br />schedules and/or procedures stipulated by WIB staff at the <br />outset of the program, transmit to state electronic reports <br />and/or forms containing all appropriate data and any other <br />pertinent information available on each participant and program <br />operations in general. <br /> <br />14. Provider shall report on progress of program to Youth Council <br />on a regular basis. <br />
