CITY OF SANA ANA <br />COMMUNITY DEVELOPMENT BLOCK GRANT <br />Scope of Work <br />Annual Accomplishment Goal <br />Total number of clients anticipated to be served during the 12-month contract period= <br />27,000 PERSONS <br />H. Total number of unduplicated Santa Ana residents to be served with grant funds during the <br />12-month contract period — 2,820 PERSONS <br />111. Description of Work - in space below, describe the program to be funded during the 12- <br />month contract period including, services to be provided, program goals, client <br />characteristics, & how grant funds will be utilized. <br />Program Goals: Provide crisis intervention and information/referral to the City of Santa Ana <br />from 8:00 a.m. to 12 midnight, 7 days per week. The populations to be served are the lower <br />income and at risk families, individuals, elderly, and youth of Santa Ana. <br />Provide an outreach service, the Good Neighbor Line, to those who are homebound due to age, <br />illness or disability; utilizing trained volunteers to call on a regular basis and maintain emergency <br />contacts. <br />Grant funds will be utilized for salaries and benefits for program services and Hotline telephone <br />lines. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served during the 12-month <br />contract period per quarter: <br />Quarter 1: <br />July 1 — September 30 <br />705 <br />Persons <br />Quarter 2: <br />October 1 — December 31 <br />705 <br />Persons <br />Quarter 3: <br />January 1 — March 31 <br />705 <br />Persons <br />Quarter 4: <br />April 1 — June 30 <br />705 <br />Persons <br />Total <br />2820 <br />Persons <br />Invoicing Schedule <br />Estimate the amount <br />of grant funds to be requested <br />during <br />the 12-month <br />contract period on a <br />quarterly basis: <br />Quarter 1: <br />July 1 — September 30 <br />S <br />1,250. <br />Quarter 2: <br />October 1 — December 31 <br />S <br />1,250. <br />Quarter 3: <br />January 1 -- March 31 <br />S <br />1.250. <br />Quarter 4: <br />April 1 —June 30 <br />S <br />1,250. <br />Total <br />S <br />5,000. <br />EXHIBIT A <br />
