CITY OF SANA ANA COMMUNITY DEVELOPMENT BLOCK GRANT Scope of Work Annual Accomplishment Goal Total number of clients anticipated to be served during the 12-month contract period= 27,000 PERSONS H. Total number of unduplicated Santa Ana residents to be served with grant funds during the 12-month contract period — 2,820 PERSONS 111. Description of Work - in space below, describe the program to be funded during the 12- month contract period including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. Program Goals: Provide crisis intervention and information/referral to the City of Santa Ana from 8:00 a.m. to 12 midnight, 7 days per week. The populations to be served are the lower income and at risk families, individuals, elderly, and youth of Santa Ana. Provide an outreach service, the Good Neighbor Line, to those who are homebound due to age, illness or disability; utilizing trained volunteers to call on a regular basis and maintain emergency contacts. Grant funds will be utilized for salaries and benefits for program services and Hotline telephone lines. Schedule of Performance Estimate the number of unduplicated Santa Ana residents to be served during the 12-month contract period per quarter: Quarter 1: July 1 — September 30 705 Persons Quarter 2: October 1 — December 31 705 Persons Quarter 3: January 1 — March 31 705 Persons Quarter 4: April 1 — June 30 705 Persons Total 2820 Persons Invoicing Schedule Estimate the amount of grant funds to be requested during the 12-month contract period on a quarterly basis: Quarter 1: July 1 — September 30 S 1,250. Quarter 2: October 1 — December 31 S 1,250. Quarter 3: January 1 -- March 31 S 1.250. Quarter 4: April 1 —June 30 S 1,250. Total S 5,000. EXHIBIT A
