CITY OF SANA ANA <br />COMMUNITY DEVELOPMENT BLOCK GRANT <br />Scope of Work <br />Annual Accomplishment Goal <br />I. Total number of unduplicated clients anticipated to be served in the Mariposa Family <br />Program during the 12-month contract period= 530_PERSONS <br />IL Total number of unduplicated Santa Ana residents to be served with grant funds during <br />the12-month contract period= 220 PERSONS 42% Santa Ana clients <br />III. Description of Work - in space below, describe the program to be funded during the 12- <br />month contract period including, services to be provided, program goals, client <br />characteristics, & how grant funds will be utilized. <br />The Mariposa Family Program is designed to meet the special needs of women who reside <br />in Santa Ana, but also serve Santa Ana families in a variety of counseling and life skills <br />education programs. These include: individual, couples, family, group, and crisis counseling, <br />substance abuse education, parenting classes (including a new series in Spanish), bereavement <br />groups, children and teens groups, occupational therapy groups, and support groups for incest, <br />grief, relationship loss, substance abusers and family members of substance abusers, and <br />overeaters. As the vast majority of the people served by Mariposa are low income, services are <br />provided on a sliding scale to enable clients to receive services according to their ability to pay. <br />Mariposa's goal is to help low-income women and their families resolve the problems they are <br />experiencing, so that they can go on to lead happy, healthy, productive lives. With respect to <br />Santa residents, our objective is to provide counseling, parenting classes, support groups, and <br />education to at least 220 Santa Ana women and their families in the course of a year. <br />Schedule of Perform ance <br />Estimate the number of unduplicated Santa Ana residents to be served during the 12-month <br />contract period per quarter: <br />Quarter 1: July 1 — September 30 55 Persons <br />Quarter 2: October 1 — December 31 55 Persons <br />Quarter 3: January 1 — March 31 55 Persons <br />Quarter 4: April 1 — June 30 55 Persons <br />Total 220 Persons <br />Invoicing Schedule <br />Estimate the amount of grant funds to be requested during the 12-month contract period on a <br />quarterly basis: <br />Quarter 1: July 1 — September 30 $ 1,500 <br />Quarter 2: October 1 — December 31 $ 1,500 <br />Quarter 3: January 1 — March 31 $ 1,500 <br />Quarter 4: April I — June 30 $ 1,500 <br />Total $ 6.000 <br />EXHIBIT A <br />
