Scope of Work <br />Annual Accomplishment Goal <br />I. Number of unduplicated Santa Ana residents to be served with grant funds during the 3-month <br />contract period= 110 PERSONS 25% Santa Ana clients <br />II. Description of Work - in space below, describe the program to be funded during the 3-month contract period <br />including, services to be provided, program goals, client characteristics, & how grant funds will be utilized. <br />Assistance in Meeting Educational Requirements: The Foundation will supply Drug Court <br />Participants with the information on educational opportunities and link participants with <br />Community College interns to assist with the admissions process and financial opportunities. <br />Supplement Program Assistance: The Foundation will complement the efforts of the Drug <br />Court team by linking participants with existing publicly and privately funded agencies that <br />identify housing opportunities, address medical/dental needs, provide job training/employment <br />information and offer assistance with transportation. <br />Alumni Continuing Care Program: The Foundation will establish a Drug Court Alumni <br />Continuing Care Program that will include graduates as well as those is the Drug Court <br />Program. This program will provide a support group for graduates and participants by offering <br />opportunities to remain in contact with the Drug Court Program by mentoring new Drug Court <br />participants, participating in panels or prevention programs and organizing activities to keep <br />graduates involved in a sober and healthy lifestyle <br />Opportunities to Satisfy the Cultural and Civic Required Activities: The Foundation will <br />arrange opportunities for the participants to experience Cultural and Civic events that <br />encourage healthy interaction with other participants and family. Some of the activities <br />include: docent led tours of the Getty Museum, Laguna Coast Wilderness Park and the Upper <br />Newport Bay Ecological Reserve. <br />Retreat: A day for the participants of Santa Ana to learn about health issues (HIV, diabetes <br />etc) possibly screening, assistance with financial and debt issues as well as job and career <br />opportunities. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served during the 3-month contract <br />period per quarter: <br />Quarter 1: July I — September 30 <br />Quarter 2: October 1 —December 31 <br />Quarter 3: January I — March 31 <br />Quarter 4: April 1 — June 30 <br />Invoicing Schedule <br />Persons <br />25 Persons <br />30 Persons <br />30 Persons <br />Total 110 Persons <br />Estimate the amount of grant funds to be requested during the 3-month contract period on a quarterly <br />basis: <br />Quarter 1: July 1 — September 30 <br />Quarter 2: October 1 — December 31 <br />Quarter 3: January 1 — March 31 <br />Quarter 4: April 1 — June 30 <br />$ <br />5,000.00 <br />$ <br />5,000.00 <br />$ <br />5,000.00 <br />$ <br />5,000.00 <br />Total $ <br />20,000.00 <br />EXHIBIT A <br />
