<br />ATTACHMENT "A" <br /> <br />ANNUAL ENVIRONMENTAL REMEDIATION CONTRACT <br />CONSULTANT RATE COMPARISON FORM <br /> <br />Consultant shall complete this form and include it along with the comprehensive fee schedule in the <br />sealed fee envelope. This comparison is based upon services required for the following sample <br />projects. This form will be used for fee comparison purposes only. <br /> <br />Environmental Remediation <br />An underground storage tank is discovered on an existing site. A Phase II Assessment is required <br />to analyze the site. In the assessment, it is determined that 3 4-inch monitoring wells will be <br />required to a depth of approximately 60 feet below ground surface. The depth to the groundwater <br />is expected to be approximately 50-feet below ground surface. The primary objective of the <br />investigation is to determine the extent of contamination in both soil and groundwater. Soil samples <br />will be collected from each boring at five-foot intervals for geological description. Selected samples <br />(a total of three soil samples) will be forwarded to a California certified laboratory for chemical <br />analysis. The samples are to be analyzed by the following EPA Methods: <br /> <br />EPA Method No. 8015 (modified) <br />EPA Method No. 6010B17000 CAM Metals, and <br />EPA Method No. 8260. <br /> <br />Based on the results of the Phase II Assessment and the monitoring wells, prepare a report for the <br />Regional Water Quality Control Board. The cost comparison should include all costs necessary to <br />satisfy the requirements stated in the RFP. Consultant may add more tasks and classifications to <br />better represent actual staff and tasks. <br /> <br />Utilitv PotholinQ <br />One mile of storm drain pipe is planned to be constructed in an arterial road. Provide all costs <br />necessary to provide 15 utility potholes. Include traffic control.
