<br />EXHIBIT A <br /> <br />SCOPE OF SERVICES <br /> <br />Consultant shall develop, coordinate and implement a "Weed and Seed Safe Haven" site <br />program at its Lyon Street Parent Center to provide computer classes for youth and their families <br />as part of the Special Emphasis Initiative, The program will provide instruction on beginning <br />Microsoft Work, Excel, PowerPoint, Access and Internet Navigation, Sessions will be taught in <br />English and Spanish, Participants will also have access to all other Center services including <br />parent education classes, youth development programs, after school services and health care <br />referrals, <br /> <br />Consultant will comply with all reporting requirements of the Executive Office for Weed and <br />Seed, Such requirements include providing monthly report to the City Community Development <br />Agency, describing the number of clients served and the type and level of services provided. <br />Consultant shall include the following documentation regarding the program: <br />Registration forms <br />Attendance sheets or sign-in sheets <br />Copies of flyers/marketing materials used to promote the program <br />Pictures of special events <br />Pre and post tests to determine program effectiveness <br />Safe Haven survey <br /> <br />COMPENSATION <br /> <br />Consultant shall be compensated as follows: <br /> <br />Computer Instructor <br />$14,00 per hour x 20 hours per week x 48 weeks <br /> <br />$13,440.00 <br /> <br />Supplies - Ink cartridges, paper, computer maintenance, etc, <br /> <br />$ 2,560,00 <br /> <br />Total <br /> <br />$16,000,00 <br /> <br />9 <br />
