<br />Attachment A <br /> <br />Instruction Sheet for the Governing Body Resolution <br /> <br />Purpose <br /> <br />Authorized <br />Agent(s) <br /> <br />Authorized <br />Agent Changes <br /> <br />The purpose of the Goveming Body Resolution is to appoint individuals to <br />act on behalf of the governing body and grant recipient. <br /> <br />The Goveming Body Resolution allows for the appointment of individuals <br />or positions. For each person or position appointed by the goveming body, <br />submit the following information, with the Resolution, to the state on the <br />applicant's letterhead: <br /> <br />[J Jurisdiction [J Telephone <br />[J Grant Program [J Fax # <br />[J Name [J Cell Phone # <br />[J Title [J E-Mail Address <br />[J Address <br />[J City <br />[J Zip Code <br /> <br />· If the Governing Body Resolution identified Authorized Agents by <br />position and/or title, changes can be made by submitting new Authorized <br />Agent information to the state, as indicated above. <br />· If the Goveming Body Resolution identified Authorized Agents by name, <br />a new Resolution is needed when any changes are made.
