Description of Work -in space below, describe the program to be funded during the 12- <br />month contract period including, services to be provided, program goals, client <br />characteristics, & how grant funds will be utilized. <br />Funded Programs / Client Characteristics: <br />- Mercy House Center, walk-in outreach center that serves Santa Ana' homeless population. <br />Clients are generally unemployed, and in need of basic emergency services. <br />Services provided: <br />- Crisis intervention / Practical counseling <br />- Food and shelter referral <br />- Clothing, hygiene prescription, transportation & laundry assistance/vouchers <br />- Assistance in obtaining vital records <br />Program goals: <br />- Increase clients' knowledge of and access to resources <br />- Improve the lives of the chronically homeless <br />Fund utilization: <br />Salaries / benefits for Program Manager position <br />Operations: Client / office supplies, rent, phone, audit fees, insurance, furniture / equipment, <br />repair / maintenance <br />
