Description of Work - in space below, describe the program to be funded during the 12- <br />month contract period including, services to be provided, program goals, client <br />characteristics, & how grant funds will be utilized. <br />The Mental Health Association of Orange County provides Essential Services for the City of Santa Ana Outreach Program. Staff <br />employed through the requested funds will provide a wide range of services to the mentally ill homeless in the City of Santa Ana, <br />including but not limited to outreach, engagement, assessment and referral services, case management, counseling, assistance in <br />accessing emergency, transition and permanent housing, transportation and employment services. <br />Since its creation in 1995, The City of Santa Ana Outreach Program has helped hundreds of mentally ill homeless adults in Santa <br />Ana to overcome barriers to service. This has been accomplished through the stationing of an Outreach Worker at existing <br />homeless service providers in Santa Ana, adding mental health expertise and additional resource knowledge to providers' existing <br />level of services. <br />Goals and Objectives for fiscal year 2005-2006 <br />GOAL: Increase access to services for the homeless mentally ill that will facilitate their transition into stable and <br />appropriate treatment programs. <br />OBJECTIVE (S): <br />1. Make contact with nine homeless Santa Ana residents, (unduplicated), each week and preliminarily screen for <br />mental illness and assess for service needs. <br />2. Actively link 200 homeless Santa Ana residents to psychosocial services annually. <br />r--x•4 <br />
