The parties entered into Agreement N-2004-045, dated May 5, 2004, (hereinafter <br />"said Agreement") by which Consultant has instructed Hip Hop I Jazz dance classes <br />through the City's leisure class program. <br /> <br />B. Consultant has formed a general partnership and desires that said Agreement be <br />amended to reflect the partnership is providing the services rather than the individual. <br />In accordance with the terms and conditions of said Agreement, the parties wish to <br />renew said Agreement for an additional one-year period. <br /> <br />WHEREFORE, in consideration of the covenants contained in said Agreement, and <br />subject to all the terms and conditions of said Agreement, as previously amended, except <br />those amended in this Second Amendment to Consultant Agreement, the parties agree as <br />follows: <br /> <br />I. The parties to said Agreement shall be amended to reflect that Consultant providing <br />services is Cathleen Forcucci Dance Academy, a California general partnership. <br /> <br />2.
