<br />'-' <br /> <br />""'" <br /> <br />EXHIBIT B <br /> <br />STATEMENT OF WORK <br /> <br />I. Scope of Work. City hereby engages contractor as an independent contractor to <br />perform the following described work, and contractor hereby agrees to perform said <br />work upon the terms and conditions here-in-after set forth. <br /> <br />A. Specifically, contractor shall perform the following services: <br /> <br />1. Contractor will provide seminars and mentoring support for eligible Rapid <br />Response participants. Such seminars shall occur in accordance with the <br />training schedule and curriculum attached hereto as the program narrative <br />(Exhibit A). Neither the seminar schedule nor curriculum may be altered in <br />any way during the term of this agreement, without prior written approval <br />given to contractor by City. <br /> <br />2. Full completion of the seminar/training program shall be defined as and <br />determined by individual participant attainment of the learning objectives and <br />demonstrated achievement of the standards for evaluation listed in the <br />training curriculum verified and approved by the City. <br /> <br />3. Contractor will utilize instructional material at reading and mathematics levels <br />no higher than those levels required for entry into those fields for which <br />training is being offered. <br /> <br />4. Contractor will provide each participant who attains full or early completion of <br />the seminar training program with a certificate of completion. Such certificate <br />shall serve as contractor's assurance to the City that the recipient of said <br />certificate has satisfied the conditions of full completion as set forth in <br />conditions identified in paragraph 2 above. <br /> <br />5. Contractor will adequately review and explain the civil rights and complaint <br />handling procedures and provide a signed copy of said procedural letter in the <br />participant's file. <br /> <br />6. Contractor will provide an enrollment orientation for each participant which at <br />the minimum includes the following information: <br /> <br />a) Program goals and objectives <br />b) length of training period <br />c) civil rights and complaint handling procedures <br />d) anticipated completion date <br />
