<br />....... GOALS AND OBJECTIVES "" <br />IMPLEMENTATION STRATEGY <br /> <br />In the space below, please list the primary goals and objectives for your program. Provide <br />projected numbers of Santa Ana residents to be served, and the kinds of services they would <br />receive if your program is funded. Also describe the specific actions and tasks that would be <br />performed on a day-to-day basis to achieve these goals. <br /> <br />(> <br /> <br />The primary goal of LSS-SC's Shared Housing Program is to help place single par~nts, seniors, the <br />disabled and other disadvautaged individuals with limited income into safe, decent and affordable <br />housing options in Orange County by matching these seekers with providers who need security, <br />help with finances or companionship. <br /> <br />Objective #1 <br /> <br />To conduct outreach services to the residents of the City of Santa Ana. <br />Task/Action: <br />. To perform four (4) presentations per month to senior centers, faith-based organizations, community <br />based organizations, hospitals, clinics, etc. <br />. Distribute 100 flyers per quarter to businesses within Santa Ana <br />. To advertise the need for housing providers in local church bulletins. <br /> <br />Objective # 2 <br /> <br />To access and intake a minimum of twenty-five (25) seekers/providers per quarter. <br />Task/Action: <br />. Screen seekers for ability to pay rent, i.e. employment, SSI, pension, etc. as well as <br />appropriateness for program. <br />. To meet with providers in their homes and screen for appropriateness for program. <br /> <br />"'"". <br /> <br />) <br /> <br />Objective #3 <br /> <br />To provide a minimum of five (5) matches per quarter. <br />Task/Action: <br />. Review applications for common interests and desires and set-up interviews for potential <br />providers/seekers. <br />. Follow-up with both provider's and seeker's within 24 hours of initial interview. <br />. Document matches on weekly reports and within provider/seekers files. <br /> <br />Objective #4 <br /> <br />75% (15) of matches will maintain housing stability for a minimum of six (6) months. <br />Task/Action: <br />. Conduct monthly follow-up calls to provider/seekers and document. <br />.
