<br />Attachment A <br /> <br />Instruction Sheet for the Governing Body Resolution <br /> <br />Purpose <br /> <br />Authorized <br />Agent(s) <br /> <br />Authorized <br />Agent Changes <br /> <br />The purpose of the Governing Body Resolution is to appoint individuals to <br />act behalf of the governing body and the applicant. <br /> <br />The Governing Body Resolution allows for the appointment of individuals <br />or positions. For each person or position appointed by the governing body, <br />submit the following information, with the resolution, to the state on the <br />applicant's letterhead: <br /> <br />0 Jurisdiction 0 Telephone <br />0 Grant Program 0 Fax # <br />0 Name 0 Cell Phone # <br />0 Title 0 E-Mail Address <br />0 Address <br />0 City <br />0 Zip Code <br /> <br />· If the Governing Body Resolution identifies Authorized Agents by <br />position and/or title, changes can be made by submitting new Authorized <br />Agent information to the state. <br />· If the Governing Body Resolution identifies Authorized Agents by name, <br />a new Resolution is needed when any changes are made.
