Application Cover Sheet with appropriate signatures: <br />Subgrantee Authorized Agent for all applications <br />Approval Authority Body for SHSP and LETPP applications <br />Core city AND core county for VASI applications <br /> <br />. Approval Authority Body member information: <br />- Name <br />Title <br />Discipline Represented <br />Mailing Address <br />Office Phone Number <br />Cell Phone Number <br />Fax Number <br />E-Mail Address <br /> <br />. Equipment Worksheet <br /> <br />NOTE: Subgrant awards will not be made until all required application <br />components have been approved by the state. <br /> <br />Submit the ISIP, Application Cover Sheet, and, if applicable, Approval <br />Authority Body membership information, to the state Office of Homeland <br />Security at the address listed at the end of Chapter One. <br /> <br />The subgrantee will receive written notice of the state's approval of the <br />subgrant application. <br /> <br />The subgrantee will receive written notice if a proposed project is <br />conditionally approved and subject to finther state review.
