Attachment A <br />State Policies and Requirements, Continued <br />Post Award Post award budget, scope and time modifications must be requested in writing <br />Modifications by the subgrantee's Authorized Agent, and submitted to the contact agency <br />listed at the end of Chapter One. The subgrantee may implement the <br />modifications, and incur associated expenses, after receiving state written <br />approval of the modification. <br />NOTE: Modifications can be requested as part of the performance report <br />process, or at any other time during the grant performance period. Failure to <br />submit modification requests, and receive written approval prior to <br />expenditure, could result in a reduction or disallowance of that part of the <br />grant. <br />Subgrantee Subgrantees must prepare and submit performance reports to the state for the <br />Performance duration of the grant performance period, or until all grant activities are <br />Reports completed and the grant is formally closed. Subgrantees must complete a <br />Biannual Strategy Implementation Report (BSIR) using the DHS on-line <br />Grant Management System, and may also be required to submit additional <br />information and data requested by the state. <br />Failure to submit performance reports could result in grant reduction, <br />termination or suspension. <br />Monitoring The state may perform periodic reviews of subgrantee performance of eligible <br />Subgrantee activities and approved projects. These reviews may include, but are not <br />Performance limited to: <br />Comparing actual subgrantee activities to those approved in the subgant <br />application and subsequent modifications, if any. <br />Ensuring that advances have been disbursed in accordance with applicable <br />guidelines. <br />Confirming compliance with: <br />— Grant Assurances. <br />— hifomration provided on performance reports and payment requests. <br />— Needs and threat assessments and strategies. <br />Continued on next page <br />FY05 Homeland Security Grant Program Page 12 <br />
