Attachment A <br />Application, Continued <br />State Required In addition to the ISIP, the state requires subgrantees to submit the following <br />Application items to complete the application package: <br />Components <br />• Application Cover Sheet with appropriate signatures: <br />- Subgrantee Authorized Agent for all applications <br />- Approval Authority Body for SHSP and LETPP applications <br />- Core city AND core county for UASI applications <br />Approval Authority Body member information: <br />- Name <br />- Title <br />- Discipline Represented <br />- Mailing Address <br />- Office Phone Number <br />- Cell Phone Number <br />- Fax Number <br />- E -Mail Address <br />• Equipment Worksheet <br />NOTE: Subgrant awards will not be made until all required application <br />components have been approved by the state. <br />Application Submit the ISIP, Application Cover Sheet, and, if applicable, Approval <br />Submission Authority Body membership information, to the state Office of Homeland <br />Security at the address listed at the end of Chapter One. <br />Application The subgrantee will receive written notice of the state's approval of the <br />Approval subgrant application. <br />Conditional The subgrantee will receive written notice if a proposed project is <br />Approvals conditionally approved and subject to further state review. Work can begin <br />on the conditionally approved project only after the subgrantee receives <br />written notice from the state indicating that the condition(s) have been <br />removed <br />FY05 Homeland Security Grant Program Page 21 <br />
