Attachment A <br />Approval Authority Body Member Information <br />SHSP and LETPP Only <br />All SHSP and LETPP applicants must submit Approval Authority Body member information as part <br />of their application package. For each member of the Authority Body, please provide the following <br />member information: <br />• Name <br />• Job Title <br />• Discipline Represented <br />• Mailing Address <br />• Office Phone Number <br />• Cell Phone Number <br />• Fax Number <br />• E -Mail Address <br />Equipment Worksheet <br />Subgrantees must submit an Equipment Worksheet with their application package. The Worksheet is <br />available at http://www.oes.ca.gov, and indicates, by project and funding source, the value of <br />equipment purchases to be made through DLA prime vendor Fisher Safety, or through the <br />subgrantee's standard procurement processes. The Equipment Worksheet is an EXCEL document <br />with formulas to automatically total amounts enter on the Worksheet. <br />NOTE: Subgrantee funds committed to Fisher Safety will be paid by the state, directly to the <br />Defense Logistics Agency, upon Fisher Safety's notification that subgrantee ordered equipment has <br />been shipped. Consequently, the funds committed to Fisher Safety will not be available for <br />subgramees to request as either an advance or reimbursement payment. <br />1, Y115 Homeland Security Grant Program Page 37 <br />
