Inc. <br />Name of Funded Program- Joseoh House/Regina House <br />Annual Accomplishment Goal <br />I. Total number of un duplicated clients (Santa Ana and Non-Santa Ana Residents) anticipated to be served by <br />the funded program, named above, during the 12-month contract period. <br />92PERSONS <br /> <br />II. Number of only unduplicated Santa Ana residents to be served by the funded program, named above, <br />during the 12-month contract period. <br />45 PERSONS <br /> <br />Program and Funding Description <br /> <br />III. Description of Work- In the space below, describe the program to be funded during the 12-month <br /> <br />contract period. <br />Joseph House and Regina House are two transitional housing programs for the homeless operated by Mercy House Transitional <br />Living Centers. Joseph House offers shelter to 20 homeless single men, including emancipated youth and veterans. Regina House <br />offers shelter to seven homeless single women with children. These programs provide housing and individualized case management <br />to help clients move to a state of self-sufficiency. Our goal for these programs to end the homeless for all who enter into our system <br />of care. In order to transition clients into a state of self-sufficiency, they must participate in the following programs: <br /> <br />1. Wellness Program - Where the objective is to improve the mental and physical health conditions of the clients. <br /> <br />2. Planning for Success Program - Where the objective is to improve the material and financial self-sufficiency of the clients. <br /> <br />3. Learning Matters Program - Where the objective is to improve the quality of life for the client because of increased knowledge and <br />modified behavior. <br /> <br />IV. Activities funded by this grant - How will ESG funds be used for the funded program? Narratively <br />describe the budget for the funded program illustrated in Exhibit B and B-1. <br />Mercy House will use the ESG funds for program and administrative staff salaries and benefits, office supplies, rent/lease, <br />communications, utilities, insurance, and facility expenses. <br /> <br />Exhibit A <br />Page I of2 <br />
